Your First Visit
The first appointment to our office is a great time to get to know your oral surgeon, our staff, and discuss your treatment goals. Your surgeon will design a custom treatment plan based on your specific oral health needs.

What to Bring
We recommend that you show up a few minutes early to fill out any necessary paperwork. Please discuss any medical conditions that you may have that you feel your oral surgeon should know about during your consultation.
Please bring the following materials with you to your first appointment:
- Your medical and/or dental insurance information, if any
- List of medications you are currently taking, including the name, strength, and dosage
- Any referral paperwork, X-rays, etc., provided by your dentist or general physician (sometimes your doctor is able to send this information to us ahead of time)
- Valid driver’s license or photo ID
- Payment information (credit card, checkbook, etc.)
Parents of Minors
If you are under the age of 18, you must be accompanied by a parent or guardian to all appointments at our office.
What to Expect
Your first appointment will involve an oral exam as well as a consultation. Your oral surgeon will thoroughly review your medical and dental history as well as your specific condition and concerns to determine the best treatment plan for you. Your oral surgeon will also answer any questions you may have. 3D scans may also be taken at this time.
3D Scanning
All of our offices utilize highly advanced 3D imaging technology to detect and diagnose problems, monitor treatment, and produce predictable surgical outcomes.
Financial & Insurance Information
Payment is due at the time of service. We work with many insurance carriers for your convenience. We will bill your insurance company and send you a monthly statement. Regardless of insurance, the patient or guardian assumes full responsibility for all fees incurred at our office, including any remaining balance that your insurance does not cover. If you have any questions about your account at any time, please contact us as soon as possible so we can clear up any misunderstandings for you.

- Most major credit cards (Visa®, Mastercard®, American Express®, and Discover®)
We also accept third-party financing plans through CareCredit®. You can learn more about how to pay for out-of-pocket healthcare expenses or easily apply online on the CareCredit website. Please note that any financing agreements should be agreed upon and discussed before your procedure date. If you are interested in third-party financing plans, contact our office, and a member of our staff will be happy to assist you.
